Transportation, Visitation, Habilitation Services
This program is designed to prevent substitute care placement, promote family reunification, stabilize foster care placements, facilitate youth development, and ensure the safety, permanency, and wellbeing of children.
Transportation
Transportation services for members of a Department of Children and Family Services (DCFS) involved families, provide transportation to designated appointments when alternate means of travel are not available. Transportation may be available for the following:
Counseling, therapy, and clinical services.
Medical or educational appointments.
Family visits.
To and from school or special events.
Court hearings.
Visitation
NYAP provides supervised visitation and transportation for children and youth experiencing foster care and their parents and/or siblings. Visits are generally held in the family home or the most home-like setting consistent with the Family Service Plan and family preference.
Habilitation
Services are designed to assist parents or other caregivers to maintain or reunify their family. These services are provided primarily in the family home and are designed to strengthen the overall capacity of parents or caregivers and to improve parenting skills..
