Home Study / Post Release Services
The Home Study and Post Release Services (HS/PRS) program is designed to offer ongoing support to help ensure the long-term safety and well-being of unaccompanied children and youth to ensure a successful and enduring transition to a family member or other sponsor.
NYAP employs a bilingual team of case managers and clinicians to provide the following services:
Home study assessment of potential sponsors’ homes to ensure safety and well-being prior to placing a child or youth.
Linkage to services such as medical, advocacy, legal, and other community resources.
Counseling and coaching to children and youth who are gang or substance involved.
Therapeutic services to assist with trauma, other mental health conditions, and family stabilization.
Navigation of the educational system including advocacy and navigating English as a second language.
Application and linkage to Trafficking Victim Assistance Programs (TVAP) as applicable.
Respond to emergent situations and other needs.
