Care Management
Care Management is a child welfare program that provides all elements of child welfare protective services to contracted entities and referral resources. We work in collaboration with the custodial county and the family to reduce risk, enhance safety, and keep the family together in their home whenever possible. Care Management draws on the strong philosophy of employing the least restrictive, community-based approach that incorporates family-centered practices and group decision-making.
Care Management Serves
Families who are in the child welfare system due to circumstances of:
Abuse.
Neglect.
Status offenses.
Delinquency.
Care Management Services Continuum
Through the Care Management Program, NYAP and our contracted community providers offer:
Home based services.
Residential treatment.
Community support services.
Therapeutic and family foster care.
Substance abuse treatment services.
How is Care Management different from other programs?
It’s not just what we do, it’s how we do it. We work in partnership with families and parents to collectively do what is best with children, youth, and the family as a whole. We give families a voice and a choice in their care and support. This program uses innovative yet realistic approaches to service delivery with measurable success and positive outcomes.
Contracted Care Management services are available throughout the Columbus, Ohio area under contract with Franklin County Children Services.
