Care Management

Care Management is a contracted child welfare program that provides all elements of protective services on behalf of children, youth, and families. We work in collaboration with the custodial county and the primary family to reduce risk, enhance safety, and keep families together in their community whenever possible. Care Management utilizes the least restrictive, community-based services that incorporate family-centered practices and group decision-making. 

Care Management services are designed for families involved with the child welfare system due to abuse, neglect, status offenses, delinquency, parental substance abuse, mental health issues, or other familial challenges that impact the safety and well-being of children and youth.  

Using a strengths-based, family empowerment approach, Care Management seeks to assist families in identifying natural supports and community services that will mitigate risk and enhance familial functioning. With the ultimate goal of keeping families intact or reunifying children and youth from foster and congregate care, Care Management is involved until a case is closed, or legal permanency is achieved.
